5.3 Team-Based Human Factors Challenges
An accident analysis was performed to determine
the cause of the error after an incident occurred. In the modern aviation
industry, it is also used to analyze databases of past accidents to prevent
accidents. Many models are used not only for accident investigations but also
for educational purposes. Under the International Civil Aviation
Convention, if a contracted state aircraft is at risk of accidents or any other
incident in another contracting state, the hazardous state will initiate an
investigation. This convention defines the rights and responsibilities of the
state. The reasoning model, commonly referred to as the Swiss cheese
model, is based on the methodology of why organizations must work together to
ensure safe and effective operations. From the pilots' point of view, in order
to maintain a safe flight operation, all human and mechanical elements must
work effectively in the system. In the causal model, the hollow represents
weakness or failure. These holes will not be directly harmful due to the
presence of protective layers. However, when all the holes are in order, the
danger will arise. There are four patterns in this model: organizational
influence, dangerous controls, preconditions, and hazardous activities.
Organizational Influence:
This layer refers to the management of climate resources, the
organization and the organizational process. For example, a crew that
underestimates maintenance costs will leave the aircraft and equipment in bad
condition.
Insecure Controls:
This layer includes inadequate monitoring, inappropriate
operation, inability to correct problems, and oversight violations. For
example, failure to provide immediate training to new employees will increase
the risk of life-threatening injury.
Unsafe activity:
Safe activity is not a direct cause of harm. There are some
prerequisites that lead to dangerous activities; An unstable mental state is
one of the reasons for poor decisions.
Mistakes and Abuses:
These are part of the dangerous
activity. Mistakes refer to people who are not able to take the right action to
get results. Abuses include acts of violation of law or regulation. These four
layers form the key components of the Swiss cheese model and risk analysis can
be performed by monitoring all of these factors.
